Reporting a Death or Serious Injury to the Ombudsman OfficeComing Soon: OMHDD will have a new website and online death and serious injury reporting forms early July.Reporting the death or serious injury of a client to the Ombudsman Office is mandatory within 24 hours. We offer an editable reporting form that you may complete on line, print out and fax into our office. Serious Injury Reporting Form - PDF Death or Serious Injury Report Fax Transmission Cover Sheet We strongly encourage reporters to complete a death or serious injury form and fax it into our office at 651-797-1950. We can receive faxes twenty-four hours a day. Faxing reports is the preferred method of reporting. It leaves you, the reporter, with a record of the information reported and the date and time reported. If you have questions, please call 651-757-1800 or toll free 1-800-657-3506 or TTY/Voice 711, and ask to speak with the Medical Review Coordinator.
